The Effect of zinc for 5 Day on endothelin-1, Tumor Necrosis Factor-Α and Clinical Improvement in Patients with Pneumonia

Abstract

Background: Many attempts to find out the prognostic of pneumonia, one off them is using biomarkers such as endothelin-1 and Tumor Necrosis Factor-α (TNF-α). The primary concern in pneumonia management is clinical improvement, commonly by administering antibiotics. Additional therapy using micronutrient like zinc can improve the outcome, therefore this study was conducted to analyse zinc supplementation for 5 days has effect on endothelin-1 and TNF-α levels as well as clinical improvement in patients with pneumonia.
Methods: This research was done through clinical test using quasy experimental method with pre test and post test design. The subjects were patients with CAP treated in Dr. Moewardi General Hospital, Surakarta, from August to November 2015. A sample of 30 patients was purposively taken, 15 patients as treatment group (supplemented with zinc) and 15 patients as control group (given empirical therapy only).
Results: Group with supplementation of zinc not significantly reduced endothelin-1 (p = 0.585) and TNF-α (p = 0.612) levels in patients with pneumonia. Post test results from control group significantly reduced endothelin-1 level (p = 0.048), however not significantly reduced TNF-α level (p = 0.232). There is no significantly difference clinical improvement in control group and zinc group (p = 0,679). There is no significant correlation between clinical improvement with endothelin-1 level (r = -0,079; p= 0,142) and TNF-α level (r = -0,236; p = 0,907).
Conclusion: Additional 5 day zinc supplementation has not decreased endothelin-1 and TNF-α levels, there also no correlation between 5 day zinc supplementation with clinical improvement. (J Respir Indo. 2017; 37: 8-14)
