Effect of Vitamin C to The Plasma Level of IL-6, Plasma Level MDA, and Length of Hospitalization Acute Exacerbation of COPD Patient

Abstract
Background: Exacerbation is an acute exacerbation that make aworsening of clinical symptoms. Increase of IL-6 plasma level and MDA plasma level due to inflamation amplification airway and lung when exacerbation is occure need hospitalization. Vitamin C is an antioxidant vitamin that also has anti-inflammatory effects and can be used as an adjunctive therapy.
Methods: An experimental study with pretest and post-test design to determine the effect of vitamin C on IL-6 plasma level, MDA plasma level, and length of hospitalization in 33 acute exacerbation of COPD patients were treated in hospitals Soehadi Pridjonegoro Sragen in October-November 2015. The sample was taken by purposive sampling divided into two groups, the treatment group (n=17) received vitamin C therapy 1×1000 mg/day and the control group (n=16) received 5 mL of NaCl 0.9 %. Levels of IL-6 plasma, plasma level of MDA was measured on admission and at discharge criteria are met. Length of hospitalization is calculated based on the number of days patients in care until discharge criteria are met.
Result: No significant found (p=0,379 )towards decreased of IL-6 plasma level between vitamin C group (-7,89±34,83 pg/mL)and control group (-17,19±38,46 pg/mL). Showed no significant (p=0,27) towards decreased of MDA plasma level between vitamin C group (-0,86±5,59umol/L) and control group (-2,81±4,31). No significant found (p=0,27) towards decreased of length of hospitalization between vitamin C group C (4,88±2,13 days) and control group (4,88±2,13 days).
Conclusion: Additional of vitamin C 1×1000 mg/day during hospitalization induce lowering of plasma IL-6, lowering plasma MDA, and shorten of length of hospitalization acute exacerbation of COPD patients. (J Respir Indo. 2016; 36: 157-66)
