Body Mass Index, Percentage of Skeletal Muscle and Albumin in Non-Small Cell Lung Cancer Patients Before and After Chemotherapy

Abstract
Background: The prevalence of malnutrition in hospitalized lung cancer is quite high but the problem is often not detected earlier. Nutrition routine examination is still rarely done due to time constraints, the condition of the patient also other things. The cause of malnutrition in cancer patients is multifactorial of which are originating from the process of cancer itself, from the effects of cancer therapy, or both. This study aimed to identify the changes in nutritional status in non-small cell lung cancer (NSCLC) patients based body mass index (BMI), percentage of skeletal muscle weight, and albumin level parameters before and after 3 cycles of chemotherapy.
Methods: Total of 33 subjects from pre-post experimental study in NSCLC patients between 2013 – 2014 in Persahabatan hospital who received chemotherapy were assessed for the changes in BMI, percentage of skeletal muscle weight by using bioelectrical impedance analysis (BIA) and albumin.
Result: There are 33 subjects, less nutritional status based on BMI 17 subjects (56.6%), based on the percentage of abnormal skeletal muscle of 30 subjects (90.9%) and hypoalbuminemia 27 (81.8%). Parameter assessment of nutritional status with BMI, skeletal muscle percentage and albumin before chemotherapy I and after 3 cycles of chemotherapy in a categorical scale not found significant changes in the value of BMI (p=1.000), the percentage of skeletal muscle (p = 1.0000) and albumin levels (p = 1.000). There are significant changes in the assessment of a numerical scale with the median value of BMI before chemotherapy I was 18.4 (16.90 to 25.00), median BMI after chemotherapy III was 18 (16.60 to 24) p = 0.000. The median value of albumin before chemotherapy I was 3 (2.80 to 4.0), the median albumin after chemotherapy III is (2.60 to 3.90) p = 0.000. Value range skeletal muscle percentage before chemotherapy I became 26.7 to 32.2 26.7 to 32.1 and the mean value of 29.58 ± 1.69 after chemotherapy with p = 0.001.
Conclusion: There are significant changes in the assessment of a numerical scale with the median value of BMI, albumin, percentage of skeletal muscle before chemotherapy I with after chemotherapy III. (J Respir Indo. 2016; 36: 73-82)
